or it may be in a separate document, or in header files, or
other means. But somehow it needs to be documented for the
programmer to use.

In any case, if you don't use it exactly as written, it won't
work - so it's as functional as a phone book.

I presume you are excluding "documented in my memory" since I
can certainly use an "undocumented" API that is only
documented in my memory (I've written one or two such APIs
that could be discovered by others, but for which the only
tangible "documentation" was the source code).
