I'd say the hand on his right shoulder belongs to the person with the blurred
out face, with his other hand on Ford's left side. The black area would appear
to be part of the hat the blurred out person is wearing. His elbow appears to
the left of the person on the left. I don't see a 5th face.

Why are faces fuzzed out... lets take a guess. First here's some information
The visible faces:
Rob Ford (that's the whole point)
Anthony Smith (dead, was shot 4 days before the star was contacted about the
video)
And in some of the versions of the picture:
Muhammad Khattak (Shot in the arm at the same time as Smith was killed,
currently appears to be in hiding considering various articles)
So now what's my guess, it is for his safety...
Also, the picture is all the press were given, but they got to see the video.
Unless you both star reporters, and the editor of gawker are lying, The video
exists. As to faking the video, well maybe
