Some news reports say the pilots were focusing on the fact they were not lined
up correctly on the runway and were working that issue and not monitoring
airspeed.
}

One cause of crashes is the flight crew focusing on a problem and forgetting
about flying. If there's a problem, then one pilot works it, while the other
flies the plane. If they're both focused on the problem, then they may miss
flight critical info. With 4 pilots on board, you'd think at least one would
notice there was a problem sooner.
