Whether it's because they fail the test on whether it's patentable subject matter or because they fail the tests on such things as obviousness:

Badly granted patents cause extensive harm while failing to uphold the exchange that is supposed to underlie the patent grant! I thought that was clear - but since it wasn't, now it's spelled out quite nicely.

And - obviously - who is responsible for triggering the invalid patents being granted?

Patent Lawyers willing to abuse the system. Patent Lawyers like Mr. Gene Quinn who once asked:

How long will it take the Federal Circuit to over-rule this ruling by the Supremes? As a result, since it's Lawyers like Mr. Quinn who are responsible for causing the mess we're in (my humble opinion of course) my beef naturally extends to them. Without their efforts we wouldn't have the problems we have today.
