Much as I would love to agree, it just ain't so. 'Release w/

All companies that produce software are glad that Microsoft has set this low
quality bar as the industry standard. If people expected all software-based
products to work 100% of the time, it would be impossible to ship non-trivial
products.

Well, there used to be alpha and beta testing. Problem is, you can't 'test'
quality into a software product, just like Deming said, you can't 'inspect'
quality into a part. I think the problem with large, complex software is the
lack of rigorous specifications. Writing good software is easy, write good
specifications is difficult. I stumbled into a good article on s/w specs here:
